Collection of personal information
We collect your personal information when you book through our online booking platform, when you write or call us, and over the course of your care at the clinic. This website has no form of its own: it directs you to those channels. The appointment request sends us:
Your first and last name
Your date of birth
Your mailing address
Your email address
Your telephone number
The reason for your consultation (information about your health condition)
Your availability for an appointment
Where applicable, the name and contact details of the person authorized to make the request on your behalf (telephone, email, relationship to you)

Use of personal information
This information is used solely to respond to your request, to offer you the service best suited to your needs and to provide your ongoing care.
When you browse this website, our hosting provider also records technical information, including your IP address, for the sole purposes of serving the pages and keeping the service secure.

By submitting an online appointment request, writing to us by email or using our services, you consent to this privacy policy.

Keep confidential the identity of the people who use our website
Never seek to identify you personally from your browsing: analytics and advertising cookies, placed only with your consent, rely on pseudonymous identifiers, never on your name
Never disclose, transmit or sell any personal information concerning visitors to our website

Disclosure of information outside Québec
To provide our services, we rely on trusted providers: our online booking platform (Jane), whose data is hosted in Canada, for appointment booking and record keeping; the hosting and delivery of this website (Cloudflare), whose servers are spread across several countries; and, where applicable, analytics and advertising tools (Google) that may process information outside Canada. Before any disclosure of information outside Québec, we carry out the privacy impact assessment required by Law 25 and ensure that your information receives adequate protection. We disclose it only for the purposes described in this policy or where the law requires it.

Your rights
Right to object and to withdraw
You may refuse to have your personal information used for the purposes stated at the time of collection.
The personal information we collect is kept in a secure environment. Everyone working for the clinic is required to respect its confidentiality. To protect your information, the measures we use include:

We undertake to maintain a high degree of confidentiality by incorporating appropriate technological innovations. No mechanism, however, offers absolute security: a degree of risk remains when personal information is transmitted over the Internet.

Retention and destruction
We keep your file for at least five (5) years following your last visit, as required by the regulation of the Ordre des acupuncteurs du Québec; it may be kept longer where this remains necessary for your care or to meet our legal obligations. At the end of that period, your information is securely destroyed or anonymized, with no possibility of reconstruction.

Confidentiality incident
In the event of a confidentiality incident involving your information, we take reasonable measures without delay to reduce its consequences and to prevent a further incident. Each incident is recorded in a register. Where it presents a risk of serious injury to you, we notify the Commission d’accès à l’information as well as the people concerned, in accordance with Law 25.

In accordance with the Act respecting the protection of personal information in the private sector, MonAcupuncteur.ca has designated Justin Lapointe as the person in charge of the protection of personal information. He ensures that best practices in managing personal information are applied within the clinic.
